2013 Alegent Creighton Health Scholarship Guidelines (To qualify, you must be a Nebraska or Iowa high school senior)
Purpose: To provide scholarship assistance for eligible students preparing for health care careers.
Eligibility: 1) NE or IA high school seniors graduating May 2013 with intent to pursue a post secondary degree in a health care field (Dentistry and Veterinary Science fields excluded). 2) Applicants must have applied to, and plan to enroll in an accredited college/university. 3) Applicants must have and maintain a minimum cumulative GPA of 3.0 or higher on a 4.0 non-weighted scale. Awards: Twelve $1,500 non-renewable scholarships will be awarded.
Application Deadline: All application materials must be received in our office by Noon on Friday, February 15, 2013. No hand written, e-mailed or faxed documents will be considered. It is not sufficient to have a postmark of February 15, 2013. No deadline exceptions will be considered. Award Notification: Award recipients will be notified via letter by Friday, March 22, 2013. Requirements: Applicants must assume responsibility for all requirements. Incomplete or inaccurate applications will not be considered.
Submit two letters of recommendation from high school teachers only. Each letter must be on high school letterhead to be considered. Letters not on high school letterhead will automatically disqualify the application.
Each letter of recommendation must be in a sealed envelope with the author’s signature placed over the envelope flap.
Send both sealed and signed letters of recommendation with your application – do not send separately. Letters of recommendation sent separately will not be considered.
Submit an official, school-issued high school transcript of grades (with official seal). Copies or unofficial transcripts will not be considered.
High School transcripts must be converted to a 4.0 scale. Conversions must be provided by the high school Guidance Counselor and not a parent. If there is no conversion by the high school official, the application will be disqualified.
Recipient Requirements: Recipients are required to assume responsibility for all requirements. Any failure to provide the necessary documentation by the stated deadline will result in forfeiture of the scholarship award.
Recipients must submit a college/university acceptance letter by May 1st.
Recipients must enroll an accredited college or university as a full-time student (12 credit hours or greater).
Recipients must submit a fall 2013 class schedule (generated from the college Registrar) from an accredited college or university by August 12th. (This must be received in our office by Monday, August 12th). Scholarship Award Schedule: Scholarship award checks will be issued the second week of September and sent to the address recorded on the application.
Priority Selection Criteria (not required but used by the selection committee to evaluate applications)
Scholarship application packet submitted completely and correctly
Quality of essay (to include presentation, grammar, spelling, and fluency)
Volunteer service hours logged at an Alegent Creighton Health facility
Participation in an Alegent Creighton Health Career Academy
Participation in an Alegent Creighton Health Care Career Camp
Volunteer and community service hours logged
Ethnic diversity
