Alegent Creighton Health Pharmacy offers $4.99 prescriptions for a 30-day supply of the listed generic medications and $12.99 prescriptions for a 90-day supply of the listed generic medications. Not all generic medications are covered under this program. The list of covered medications and the prices charged for medications are subject to change at any time without notice. Alegent Creighton Health Pharmacy reserves the right to modify or discontinue the program or modify the terms and conditions that apply to the program at any time without prior notice.
$4.99 prescriptions include up to a 30-day supply of covered medications at commonly prescribed quantities. $12.99 prescriptions include up to a 90-day supply of covered medications at commonly prescribed dosages. Physician permission may be required to change a 30-day prescription to a 90-day prescription.
To qualify, prescriptions must be filled at an Alegent Creighton Health Pharmacy location. All prescriptions must be picked up at an Alegent Creighton Health Pharmacy location to qualify for the program.
