Acute Inpatient Treatment

Client:
Patients with severe psychiatric needs.

Hours:
The units operate 24 hours a day, seven days a week.

Service Description:
Stabilization of the patient presenting symptoms and restore him/her to an improved quality of life.

Follow-up Care:
After initial evaluation by the physician, the interdisciplinary team will further assess the patient. These professionals collaborate with the patient and the patient’s family to draft a plan of care to begin the healing process. Progress is monitored closely, and when appropriate, the patient proceeds to the next level of care.

Service Goal:
When a patient has been stabilized and achieved the maximum benefit from acute care, follow-up care will be offered on an outpatient basis. Alegent Creighton Health provides follow-up care through its outpatient programs in many convenient locations.

Treatment Team:
The interdisciplinary team provides medical, biological, social, spiritual, behavioral and emotional support.

Referral Process:
Referrals to acute psychiatric services can be made directly by a psychiatrist and/or a physician, by the Department of Social Services, schools, family, courts, therapists, psychologists, Emergency Departments of Alegent Creighton Health hospitals or clergy. Community Support Services

Client:
Patients who need assistance in living a productive life in the community.

Service Goal:
For patients to learn the skills necessary to remain out of hospital care, institutional care and support integration back into the
community.

Service Description:
A variety of community support services help patients live a productive life in the community. Home care,
educational groups, support groups, home visits, leisure skills, time management, life group and other activities help to keep individuals stable and productive.

The staff is in communication with referral sources on an ongoing basis as the need exists.

Treatment Team:
Community support specialists coordinate with members of the patient’s treatment team, including psychiatrists, therapists, psychologists, nurse practitioners, the patient and his/her family.

The M.D., Ph.D., therapist, IP staff and partial staff make referrals. The patient must be in active treatment and diagnosed with severe,
persistent mental illness. Calls to Community Support Services should be initiated by calling (712) 328-2609.

Outpatient Services

Client:
Persons experiencing mental health issues that are disrupting interpersonal, family or work life.

Hours:
Hours vary depending on clinic. Consult the Alegent Creighton Health Psychiatric Associates locations on Page 3.

Service Goal:
Restore and stabilize the client to his or her previous level of functioning.

Service Description:
Provide short-term, problem-focused therapy services or recommend longer courses of treatment for clients requiring additional assistance in achieving stabilization. Treatment modalities include individual, group, marital, family, psychopharmacological and others based on individual assessment.

Follow-up Care:
In therapy, clients learn improved life skills to promote their mental health and wellness. Often, clients are able to maintain their mental health without continuous follow-up. Some clients may be referred to community support groups to be more self-reliant and learn additional life skills.

Treatment Team:
The team may include physicians, nurse practitioners, psychologists (when indicated), therapists, client and significant others, as appropriate.

Partial Hospitalization/Intensive Outpatient Programs

Adult patients who are in acute distress or have just completed an episode of acute inpatient care may participate in this program if they are able to function independently and participate actively.

Hours:
The partial programs are open during the day from Monday through Friday. Specific hours vary depending on the program in which a patient participates.

Service Goal:
This is a short-term, ambulatory, active treatment program to help each patient regain a balance in body, mind and spirit through
therapeutically intensive and structured clinical services.

Service Description:
The Partial Hospitalization Program provides crisis stabilization through daily, intensive programming. All patients referred are
screened to determine the appropriateness of intensive outpatient care. Upon admission, patients and staff members together develop an individual treatment plan. Patients are treated with an interdisciplinary approach.

Follow-up Care:
When the patient has met treatment goals and no longer needs the partial level of care, follow-up outpatient care will be arranged. Alegent Creighton Health provides outpatient services in many locations throughout the Omaha/Council Bluffs metropolitan area.

Treatment Team:
Professionals on this team may include a psychiatrist, therapist, registered nurse, and therapeutic recreational
specialist. Additional members may include pastoral care or a dietician. Other adjunct treatment team members are available as
needed.

Referral Process:
Patients are generally referred to the partial program following a brief stay in the inpatient area, or from their outpatient psychiatrist or
therapist. Referrals may be made by calling (402) 572-3270 in Nebraska or (712) 328-5285 in Iowa.

Psychiatric Home Care

Client:
Patients of all ages having primary or secondary mental health diagnoses.

Hours:
These services are available as needed.

Service Goal:
A comprehensive evaluation of the patient is conducted in the home and in community environments to help the patient increase independent living skills and move toward mental wellness.

Service Description:
The staff teaches a patient to cope with stress through effective communication skills and by emphasizing
positive coping strategies.

Assistance is also provided with medication, diet and in helping the patient monitor symptoms to maintain
wellness.

Follow-up Care:

The program potentially enables earlier discharge from psychiatric inpatient stays and allows for the continuation of care in the community.

Treatment Team:
Working closely with the primary care physician, each patient’s care is coordinated through our psychiatric team, including nurses, medical social workers, physical, occupational and speech therapists, home health aides, and secondary care providers involved with the patient.

Referral Process:
Their physician or other healthcare workers refer patients, all of whom must be under a physician’s care.
