The Diabetes Management Program is a physical activity and education program
designed to help adults with type 2 diabetes reclaim their health.
It is a grant-funded pilot program sponsored by Blue Cross Blue Shield and
developed by the YMCA and Alegent Creighton Health. The intent of the program
is to help people who are either living with or at risk for developing diabetes
with the most effective treatment in the region.
Cut diabetes risk and gain tools for healthy living
The FREE 13-week program helps our target audience through a unique approach
to both diet and exercise.
Participants will work with trained staff from the YMCA and Alegent-Creighton
Health to safely achieve their goals. This program includes a family YMCA membership
for the length of the program. Participants will receive individual attention
in a group setting, creating a safe and fun environment. Apart from increasing
physical activity, participants will be educated on how to improve other aspects
of their current lifestyle to better manage their diabetes.
The program will include 2 weekly physical activity sessions, led by certified
YMCA trainers, and a weekly education session, led by Alegent-Creighton Health
diabetes educators and registered dietitians.
Participants will receive a thorough pre-screening and post-screening. The
screenings will include:
- Body Composition Assessment (via bioelectrical impedance)
- Lab wrk (including minimally cholesterol screening and A1c)
- Fitness assessments
An abbreviated “Fit Check” will also be performed half-way through the program
to track progress and keep participants motivated toward reaching their goals.

Sessions will begin this summer! A doctor’s referral will be necessary before
