An efficient network of nursing care by a team of Nurse Practitioners that compliments and coordinates with the care patients receive from their skilled nursing facility. The Alegent Health Nursing Home Network focus is to promote quality and function for the patient, as well, as support the family and skilled nursing staff.

Choosing Skilled Care

During a skilled nursing stay, the patient is provided with intensive rehabilitation services to assist them in regaining strength and function. Discharge plans from the skilled nursing facility often hinge on the progress the patient makes. The Nursing Home Network can help ensure your loved one is provided with the best opportunity to maximize their rehabilitation.

The Network Team Provides:

  • Highly skilled nurse practitioners available 24/7 who make regular visits to each of the member facilities. Our nurse practitioners work directly with each patient and family to develop, coordinate and execute a specific plan of care.
  • Coordinated care during the transition from the hospital to the skilled care facility and back home.
  • Assistance with long-term care arrangements.
  • Explanation of resources available in the community.
  • Physicians who serve on an advisory board and provide back-up coverage to the nurse practitioners.
  • Social worker support for guidance in care options with a focus on quality and function for the patient.

Referrals or requests for information may be made by the hospital discharge planners, nursing home admission coordinators and/or the patient’s primary care provider. Patients and families can also contact the Network team for more information.

Referrals

The Nursing Home Network services can be accessed through the hospital discharge planners, nursing home admission coordinators and/or the patient’s primary care provider. Patients and families can also contact the Network team for more information.

Care provided by Network nurse practitioners is covered by Medicare and most private insurance. Participation is completely voluntary and services begin as soon as the Network team is contacted.


Outcomes

Network patients have demonstrated the following outcomes:

  • Better pain control
  • Lowered re-hospitalization rates
  • Fewer emergency room visits
  • A reduction in scheduled medications
  • Decreased depression scores
