Choosing Skilled Care

During a skilled nursing stay, the patient is provided with intensive rehabilitation services to assist them in regaining strength and function. Discharge plans from the skilled nursing facility often hinge on the progress the patient makes. The Nursing Home Network can help ensure your loved one is provided with the best opportunity to maximize their rehabilitation.

The Network Team Provides:

  • Highly skilled nurse practitioners available 24/7 who make regular visits to each of the member facilities. Our nurse practitioners work directly with each patient and family to develop, coordinate and execute a specific plan of care.
  • Coordinated care during the transition from the hospital to the skilled care facility and back home.
  • Assistance with long-term care arrangements.
  • Explanation of resources available in the community.
  • Physicians who serve on an advisory board and provide back-up coverage to the nurse practitioners.
  • Social worker support for guidance in care options with a focus on quality and function for the patient.

Selecting a skilled care facility for a loved one is a difficult, but important decision. When determining which skilled care facility will best meet your loved ones needs, you may want to consider:

  • Choosing a facility that is close to your home and/or workplace – frequent visits
  • Touring more than one facility to compare care and services.
  • This will be your loved one’s home for the next few weeks – bring items from home to comfort them during their stay.
